The Goldfish Club Membership Information
Printing instructions: click on the picture above to take you to a full size file of the form. I have made the full size form the appropriate size to print on one page on my home printer when viewed in Internet Explorer 6.0. To ensure that the form prints on one page on your printer, I would suggest saving the JPG file to your hard drive and using a word processing or image editing program to obtain the correct size with minimal margin space. Please note that payment for membership fees, subscriptions, and club items is to be made in British pounds only.
